LAY SUMMARY:

Stem cell transplantation is a highly complex task involving many specialists.  These days it usually consists of harvesting of the stem cells from the blood of a donor (instead of bone marrow).  The subsequent harvesting (freezing) of such a product as well as its potential enrichment with growth factors and eventual readministration of the product to the patient.  Allogeneic transplantation refers to donation of the cells from someone other than the patient while autologous transplantation usually describes the preparation of a cell product from the patient himself.  The latter therefore primarily allows for administration of higher doses of chemotherapy and does not by strict definition consist of a “transplantation”.  Dr. van Besien’s major contribution has been to reorganize our transplant and lymphoma program and to more than double its size.  He has done so by focusing on several areas of research interest, in particular the allogeneic transplantation of patients with lymphoma.  This is an interesting concept in that lymphomas are, to some degree, quite responsive to chemotherapy.  Therefore the administration of higher doses of chemotherapy in this group of more chemotherapy-responsive tumors is more successful than it is in more chemotherapy resistant types of tumors.  However it requires careful management of potential complications and in the case of allogeneic transplantation “graft vs. host disease (GVHD)”.  Dr. van Besien has carefully nurtured a team of transplant experts that has expertise in all aspects of transplantation and in the areas of lymphoma and leukemia therapy.  Together they have defined an outstanding program within our institution and this was made possible by Dr. Koen van Besien’s efforts.
