LAY SUMMARY:
|
Dr.
Stadler is a Clinical Scholar who investigates novel therapies in patients
with advanced malignancies of the genitourinary track. This includes tumors
of the kidney, ureters, bladder, prostate, and testis. Within this field, Dr.
Stadler has focused predominantly on the development of novel agents in renal
(kidney) cancer. He has also been active in the area of advanced prostate
cancer that is no longer responsive to manipulation with hormones. His work
has particularly focused on ÒtargetedÓ agents. These targeted agents have
been developed in distinction from previous classes of ÒcytotoxicÓ agents
(also referred to as chemotherapy) which generate damage to a variety of
tissues in addition to tumor cells. Targeted agents, on the other hand,
explore our increased understanding of carcinogenesis and tumor cell survival
mechanisms and are intended to act more specific cells against cancer cells.
In addition, they frequently are thought to not result in the death of the
cell, but instead to limit the potential of the tumor for further growth. Dr.
Stadler has investigated several such agents in kidney cancer successfully
and is a well known and highly respected investigator on a national and
international basis. His work has lead to our improved understanding of the
role of such agents as the antiangiogenic agent, Carboxyaminoimidazole (CAI)
and, more recently, the highly promising agent, sorafenib. The latter is
likely to receive FDA approval for use in patients with kidney cancer and Dr.
Stadler and his team have made important contributions to this process. Dr.
Stadler has also greatly contributed to our clinical trialsÕ methodology to
evaluate targeted therapies that have a cytostatic effect on tumors. He
developed, in collaboration with Drs. Ratain and Rosner, a novel clinical
trials design that allows for a clever enrichment of a study population that
is most likely to benefit from the cytostatic agent. This trial design is
referred to as the randomized discontinuation trial and is increasingly being
used in clinical trials practice. A
third area of interest is the development of surrogate markers of
antiangiogenic therapies. This is necessitated by our inability to reliably
measure an effect of an agent against a tumorÕs ability to form new blood
vessels. In theory, a tumorÕs inability to form additional blood vessels
should impede its growth, but would not necessarily lead to tumor shrinkage.
However, even stopping a tumor from growing further, could lead to a
substantial survival advantage, particularly when used early in the therapy
of a patient. In order to more reliably measure the antiangiogenic effect of
a new drug, additional surrogate endpoints are needed. Dr. Stadler and
colleagues in the Department of Radiology are currently evaluating the use of
MRI technology (dynamic contrast enhanced MRI or DCE-MRI) to assess its
ability to detect early on changes in the vascular make up of a tumor. Finally,
Dr. Stadler runs one of our busiest clinical practices. He has received
peer-reviewed funding and successfully leads our GU Oncology Program at the
University of Chicago. |
