EXAMPLES OF E-FORM ENTRIES

While everyone knows how to write a research statement for reappointment or promotion, many find the educational, administrative, and clinical activity statements difficult to write.  The following links lead to model educational, administrative, and clinical activity statements.  The model statements are from the actual e-forms of faculty members who were recently reappointed as assistant professors.  While these materials are ordinarily confidential, the faculty members who wrote them volunteered to waive confidentiality to benefit future faculty who need to write such statements.  As these examples will reveal, e-form entries vary greatly in their content and detail.  Faculty seeking reappointment or promotion should not imitate these word for word, but use them to gain a sense of what the higher levels of review are looking for.
