Electronic Forms for
Reappointment, Promotion, and Tenure of Current Faculty and Research
Associates with Parenthetical Rank, and New Appointments of Faculty and
Research Associates at the Rank of Associate Professor or Professor Following
the links below will download Microsoft Word documents to be used to transmit
much of the information that the Dean and Provost require for appointment,
reappointment, promotion, and tenure decisions. These replace the paper forms previously in use. Normally current faculty who are
candidates for the proposed personnel action will respond to the BLUE items and forward the form electronically to
the department proposing the action, the department proposing the action will
respond to the RED items and forward the form
electronically to the Office of Academic Affairs, and the advisory committee
(COROAP or COAP) will respond to the GREEN
items. Additional
items may be required, and are specified on the corresponding forms and
instructions. Instructions/cover
pages should now be downloaded separately from the forms themselves. REAPPOINTMENT
